Sounds like you need something like a PCF control you can put on a form. I found this free tool (https://pcf.gallery/timer-control/) - it is not exactly what you want, but you could get a developer (or the author) to modify it for you.

I am looking for a way to track time spent on a Case in Dynamics CRM. I found Timer that I can add that is related to SLAs but that only counts down. I would like to use a timer to count upwards when a Case window is open. I know I can tally duration from the activities, but I am looking to automate tallying how long the Case window was open while a user was working on it. Is there a way to have a running clock that keeps track of how long the Case Window is open?
